Format of card. The face of the identification card for persons with disabilities shall prominently bear wording selected by the department that is indicative of the presence of urgent medical information on the reverse of the card. On the reverse side of the identification card shall be a space within which the department shall enter such medical information as the applicant may request. The department may print the urgent medical indicator and wording on the reverse of any driver’s license or identification card upon receipt of the required documentation from the person requesting its inclusion. ## History Code 1981, § 40-5-173, enacted by Ga.
